Please only submit ONE form!

Include all of your total allotted credits on this form.

CREDITS:

Small Farm Box (includes 9 credits)

Medium Farm Box (includes 19 credits)

Large Farm Box (includes 39 credits)

You’ll receive a set number of credits based on your box size, which you can use to choose food items from the list. Each item is worth a set amount of credits indicated next to the item. Instructions on how to calculate credits below.

SELECTION PREFERENCE:

You can select as many options as you like as long as you stay within your total credit limit.

ADDITIONAL CREDITS:

If you want additional credits OR to upgrade your plan, please send us an email at [email protected]. Your card is automatically billed every month.

DELIVERY:

Your Farm Box will be delivered to your door once per month every 1st Wednesday of the month:
